How to Apply for Assurance Wireless

You can apply for Assurance Wireless three ways: online on its website, by phone at 1-888-321-5880, or in person with an Assurance representative in your neighborhood. Behind every route is the federal Lifeline program, so you qualify the same way as with any free government phone company: through a program like SNAP or Medicaid, or with income at or below 135% of the poverty guidelines.

Applying is free. Assurance says its California application takes about 7 to 10 minutes, and having your benefit letter or ID ready makes any route faster.

This guide walks through each route, the documents Assurance asks for, the extra steps in California, how to check your application status, and what happens after you are approved. For an overview of the company first, read our Assurance Wireless review.

Before you apply: a quick checklist

  • You live where Assurance works. It serves over 40 states, not Oregon. Enter your ZIP code on its site to check.
  • You qualify for Lifeline. Someone in your household gets SNAP, Medicaid, SSI, Federal Public Housing Assistance or Veterans Pension, or your household income is at or below 135% of the poverty guidelines ($21,546 a year for one person in 2026 in the 48 states and DC). See our Lifeline eligibility guide.
  • No one else in your household has Lifeline. Only one Lifeline benefit is allowed per household, with any company.
  • You have your details ready. Full name, date of birth, home address, and the last four digits of your Social Security number (or a Tribal ID number).
  • You know your phone plan. Decide whether you will bring your own phone or buy one from Assurance. Outside California, Assurance does not give a free phone.

Assurance is not available in every state; it serves over 40 and left Oregon in 2022. Its site has a ZIP code checker.

For seniors and anyone who finds online forms hard, the phone route is often easiest. Assurance’s Customer Care line takes applications over the phone. Have your benefit letter or ID beside you when you call, and ask the agent how to send a copy if they need proof.

Documents Assurance Wireless asks for

You may not need any documents. If the National Verifier can confirm your eligibility from government records, you are approved without uploading anything. If it cannot, you will be asked for proof.

If you qualify throughBring one of these
A program (SNAP, Medicaid, SSI, public housing, Veterans Pension)Benefit ID card with your name and an issue or expiration date, or an award letter or benefits statement with your name and address dated within the last 12 months
Income, using one documentLast year’s tax return; a current income statement from your employer; a Federal General Assistance participation letter; or a divorce decree or child support document
Income, using monthly statements3 consecutive months of statements from the last 12 months: pay stubs, Social Security statement, VA benefits, retirement or pension, or unemployment or workers’ comp
Both routesProof of identity and proof of address

Tips that help documents get accepted the first time:

  • The name on the document should match the name on your application.
  • Make sure the date is visible and within 12 months.
  • Take the photo in good light, with all four corners showing.
  • Cover account numbers that are not needed, but leave your name, address and dates visible.

Our guide to Lifeline documents shows what each document must include so it is accepted the first time.

Step by step online

  1. Enter your ZIP code and email on Assurance’s site to check availability.
  2. Fill in your name, date of birth, address and the last four digits of your Social Security number (or Tribal ID).
  3. Choose how you qualify: program or income.
  4. Your eligibility is checked against federal records through the National Verifier. If it cannot confirm you, upload your documents.
  5. Choose your phone option: bring your own phone, buy a low-cost phone, or, in California, receive a free basic smartphone.
  6. Once approved, activate your SIM, eSIM or new phone.

What to expect at step 4: you may be confirmed right away through database checks. If you are asked for documents, the review takes longer. If the Verifier finds that someone at your address already has Lifeline, you may be asked to fill out the USAC Lifeline Household Worksheet to show that you share no income or expenses with them.

At step 5, Assurance’s phones for approved customers start at $31.99 (Artia SP555) and go up to $129.99 for an iPhone SE, plus tax and shipping. To bring your own, check that it works first with Assurance’s IMEI compatibility checker.

California applicants

assurance wireless california lifeline

Since February 1, 2026, Californians apply separately for California LifeLine and federal Lifeline. Assurance lists its California steps as: give your information (about 7 to 10 minutes), choose a phone, pass the National Verifier check, send proof if asked, then allow about 7 to 10 days for processing before activating. California customers get 12 GB of high-speed data.

Some people search for an “Assurance Wireless California Freedom plan.” Assurance’s California page calls its plan California LifeLine, with unlimited talk and text, 12 GB of high-speed data and a free basic smartphone for eligible customers. When it is time to renew, the California LifeLine Administrator contacts you (1-877-858-7463).

Texas applicants

Texas runs its own Lifeline process instead of the National Verifier. Assurance’s renewal page points Texas customers to the Texas Low Income Discount Administrator (LIDA) at 1-866-454-8387 or texaslifeline.org. Apply with Assurance as usual, and expect your eligibility to be checked through the Texas system.

How to check your Assurance Wireless application status

  • Call Assurance Customer Care at 1-888-321-5880 (daily 7 a.m. to 7 p.m. Central). Have your name, date of birth and address ready.
  • Check the National Verifier. If you started an application at getinternet.gov, you can log in there to see whether you are approved or if documents are needed. Our guide to Lifeline application status shows how.
  • Watch your email and texts for requests for documents. A missing document can hold up your application.
  • Lifeline Support Center: 1-800-234-9473 for questions about the National Verifier itself.

If you were denied, the letter or screen will give a reason, such as income over the limit, a duplicate at your address, or a document that could not be read. Our guide to a denied Lifeline application explains how to fix each one.

After you’re approved

Your plan starts with unlimited talk and text and 10 GB of high-speed data (12 GB in California and on Tribal lands). If you want more, the Data Peace of Mind add-on is $1 a month or $10 a year for 15 GB.

If you already have Lifeline elsewhere

You can move your benefit to Assurance with a Lifeline transfer. Only one Lifeline benefit is allowed per household. If your application is stuck, Assurance Wireless customer service lists every number and hours.

For the general National Verifier process behind every company, see how to apply for Lifeline. If you qualify through SNAP, a current benefit letter is usually the fastest proof. To compare Assurance with other companies first, see our Lifeline providers guide.
